Pre-Qualification Form Training

The Pre-Qualification form is an essential tool for all Agents. The key is to understand the form in a way to effectively explain and answer any questions your potential clients may have. In order to do this, the first step is to thoroughly learn the Pre-Qualification form yourself!

Loan Pre-Qualification Form Guide

The following explains the various sections of the Pre-Qualification form.

Borrower Information:

  1. Borrower: Please provide borrower’s FULL GIVEN NAME, including middle initial and suffix (if applicable).
  2. SSN: The Borrower’s Social Security Number is ESSENTIAL to begin the processing of the loan. Without it, we cannot pull a credit report!
  3. DOB: Make sure the Borrower’s Date of Birth matches with the age that they write in the next box over!
  4. Current Home Address: IMPORTANT! Please have the borrower list ANY AND ALL properties that they have lived at in the past 24 MONTHS. We need a full two-year history, totaling 24 months. (No, two years does not mean 23 months!) Please provide a FULL 24 MONTH HISTORY.
  5. Does your borrower currently own a property? If yes, what is their mortgage payment?
  6. If your client currently rents a property, please provide the amount that they currently pay, as well as any other amount that they paid for rent within the last two years. REMEMBER: Rental history will be verified, so make sure your borrower provides numbers that can be verified via cancelled checks or a property management company.
  7. Please make sure the borrower fills out employer name and address. If there are multiple addresses please provide all information. (This happens in cases of a difference in addresses between where a borrower physically works and the address of the corporate headquarters that appears on their pay-stubs and w2s)
  8. Position: This is the position title that can be VERIFIED by your borrower’s employer(s). The lender will call the employer of your borrower directly to verify the position title and length of employment. REMEMBER: Position Title will be verified, so make sure your Borrower provides their correct and current Title.
  9. Years on This Job: Similar to the rental history, please have your borrower complete a full 24 month employment history. Length of employment must cover at least 24 months, but please provide all starting dates (if multiple jobs) and (if applicable) ending dates of previous jobs held within the past 24 months. REMEMBER: Length of employment will be verified, so make sure your borrower provides accurate dates for all current and previous employment.
  10. Self-Employed: If your borrower is self-employed, indicate yes in the self-employment space.
  11. Previous Employer: If your borrower has less than 24 months with his current employer, indicate who his previous employer was and number of years worked there.
  12. Monthly Gross Income: Provide your borrower's GROSS (before taxes and deductions) monthly income.

Co-Borrower Information:

  1. Please complete information for any co-borrowers of the loan, similar to the borrower information.
  2. REMEMBER: If the Co-Borrower is not married to the main Borrower, have them complete a separate pre-qualification and borrower signature authorization form.

Loan Information:

  1. Purpose: Indicate whether loan is for Purchase, Refinance Rate & Term, or Refinance with Cash Out.
  2. Property Type: Indicate the type of property being purchased or refinanced. This information will help in the pricing of your borrower’s loan. Lenders price Single Family residences differently from condominiums and town homes, 2-4 units, etc.
  3. Occupancy: Indicate whether the home is to be a primary residence, a second home, or an investment property.
  4. Preferred Loan Program: If your borrower has a loan program preference - FHA, 203k, VA, 30 year fixed, modification, etc., indicate it here.

Purchase Information (for purchase loans only):

  1. Minimum Price / Maximum Price: Some borrowers have a specific price range for the property they are interested in.
  2. Maximum Monthly Payment Desired: It is also helpful to ask your borrowers what is the most they can afford in terms of their monthly mortgage payment.
  3. Location / Areas Interested in: Indicate the cities or zip codes your borrower is interested in.
  4. Features: Indicate number of bedrooms, bathrooms, and approximate home size.
  5. Down Payment: Indicate the amount the borrower wishes to use as a down payment. This could be as little as "0" on a VA loan, 3.5% on a FHA loan, or more on other conventional Fannie Mae loans.
  6. Source of Down Payment: Where are the down payment funds currently being held?
  7. 100% Financing Request: If your borrower is eligible for a VA loan and wishes to go with 100% financing, indicate that here.

Refinance Information:

  1. Address of Property Being Refinanced: Make sure that your borrower(s) disclose a complete address including zip code and unit number (if applicable).
  2. Current Market Value: Please indicate the current value of the property.
  3. Mortgage Balances: Indicate the current mortgage balance of the 1st mortgage and the 2nd mortgage (if there is one). If these amounts are unknown, they can be found on the current mortgage statement.
  4. Cash Out Desired: Indicate the amount of cash-out desired (if any).
  5. Pre-Payment Penalty: Indicate if there is any known pre-payment penalty in place on the loan.

Source of Funds:

  1. In this section please have your borrower(s) provide all liquid asset information. Liquid assets include, but are not limited to: Checking/Savings Accounts, 401k’s, Stocks, Bonds, Life Insurance Policies.

Certification:

  1. Please have your borrower(s) sign and date to certify that the information provided is accurate.

Borrower Signature Authorization:

  1. Please print your borrower's names in item #1, and have them sign and date at the bottom. This form allows us to move forward with the loan approval process and to run their credit report.
